Fill out form completely, and follow the directions below.

How to Submit Form:

1) Fill out every form field as needed,
2) ‘Save’ the document once completed,
3) Email the form directly to odessasoftball19@gmail.com
4) After you email the completed form, your entry fee may be submitted by Venmo to 432-310-5510, Bunny Adams-1.

Remember, a limited number of teams will be accepted!

Print out the team roster form and have each player personally sign and provide full information on it.

The City of Odessa Assumption of Risk form must also be printed out and completed by every player once each year.

– Assumption of Risk Form –

Deliver the completed roster and Assumption of Risk forms to the complex Saturday, March 6, from 3-5pm or Sunday, March 7, from 2-4pm. 

If registration form and fee have not been submitted electronically and there are still available spots, a team may enter in person at that time. No personal checks or credit/debit cards will be accepted.

Make note of the date and time for the mandatory managers meeting. A coach or responsible representative from each team is required to attend, pay attention, and inform the rest of the team about everything discussed. Failure to attend the meeting will result in the team being dropped from registration. 

Also, any team not submitting complete entry fee, completed roster, and all Assumption of Risk forms before 4pm Sunday, March 7 may be dropped from registration.
